If your child will be 4 years of age on or before September 1, 2017,
you may complete the registration process for the
2017-2018 PRE-K PROGRAM at ECLC.
PROCESS FOR REGISTRATION
Pre-K application packets available at the ECLC Front Office.
Times: Monday – Friday; 9:00 a.m. – 2:00 p.m.

Complete ALL parts of the application.
Only a parent or legal guardian may register a child.
BEFORE YOU CAN OBTAIN A PRE-K PACKET, YOU MUST
PROVIDE INCOME VERIFICATION SUCH AS THE TWELVE MOST
CURRENT PAYCHECK STUBS OR 2016 INCOME TAX RETURN.
ADDITIONALLY, THE FOLLOWING OTHER ITEMS
ARE REQUIRED BEFORE AN
APPLICATION CAN BE RECEIVED:
(You will not receive an application packet until all items are received.)
1 Proof of Residency (such as electric, water, or gas bill, etc.)
(You must reside in the Sulphur Springs Independent School District.)
Parent or Legal Guardian’s Photo ID
Child’s Official Birth Certificate
Child’s Social Security Card
Child’s Up-to-Date Immunization Record
Dr.’s Notes Concerning Allergy/Health Needs
Custodial Documents or Court Orders, if applicable
Your registration packet must be filled out completely!

ANTES DE QUE USTED PUEDA OBTENER UN PAQUETE DE PRE-K,
USTED DEBE PROPORCIONAR VERIFICACIÓN DE INGRESOS COMO LOS
DOCE TALONES DE PAGO ACTUAL O DEVOLUCIONES DE IMPUESTOS 2016.
ADEMÁS, OTROS DOCUMENTOS QUE SON REQUERIDOS ANTES QUE LA APLICACIÓN SEA RECIBA COMO LOS SIGUIENTES:
